Complete Form 244 in the address change section. Provide the U.S. Postal Service mailing address to include the house or box number, street, city, state and ZIP. Please do not abbreviate the city. For example, KCMO should be Kansas City, MO. Have the policyowner(s) sign in the appropriate signature section. A phone call from the active agent or policyowner is also acceptable.
