When a policy is lost or destroyed, a certificate of coverage can be requested and issued. Complete Form 244 pertaining to the lost policy. Have the form signed by the Policyowner, and Assignee if the policy has an assignment. A phone request from the active Agent or Policyowner is also acceptable.
