Complete Form 244 pertaining to Name Change or submit written notification from the person whose name has changed. We require the old name, new name, reason for the name change, signature and date. There are exception cases and those may require additional documentation; refer to the chart below.
REASON | WHAT IS NEEDED |
Company Name Change | Copy of the Affidavit reflecting approval of the change. The Affidavit must be signed and dated by the Secretary of State, of the State where the company is located. |
Citizenship | Copy of the citizenship papers showing the change. |
Beneficiary Name Change | A new change of beneficiary form. |
Divorce | Change of name form and a copy of the divorce decree showing the change, signed by a judge. |
Adoption | A change of name form and a copy of the adoption papers signed by a judge. |
Error At Issue | If the name was spelled incorrectly when the policy was issued and was due to a Home Office error and the policy is more than two months old, notify Customer Services and they will handle the correction. If the incorrect spelling was not due to a Home Office error, a copy of the birth certificate is needed. If a birth certificate is not available, a copy of a driver’s license or baptismal record will be accepted. |
