Have the Assignee (usually a bank or financial institution) complete Form 224.
In lieu of this completed form, we will also accept a letter from the President or Vice President, on official bank letterhead, requesting the release. Ensure the policy number and Insured’s name is also on this letter. The signature of the President or Vice President is required on the letter with their title clearly stated.
