If Evidence of Insurability is required, be sure to have the Insured complete the entire application. Obtain the signature of all proposed Insureds older than 18 years of age and the Owner (if different).
Submit the form and a payment equal to the cost of the rider or benefit. Submit to New Business/Reissue at the Home Office.
