Complete Change Application Form A169 (see State exceptions for application A169 maintained in Sales Forms on www.kclic.net).
Obtain the appropriate signature of all proposed Insureds older than 18 and the Policyowner. Submit the request to New Business/Reissue at the Home Office.
Evidence of insurability is required for the following changes. Submit appropriate evidence of insurability pages found in the change application.
